The game's story centres on the discovery of a substance called Mojo, which Doctor Neo Cortex intends to use to transform the inhabitants of Wumpa Island into an army of loyal mutants known as "Titans". Crash must stop Cortex using the "overlap" technique to take control of them and destroy Cortex's Titans while collecting the mojo.

Le gameplay, bien que toujours centré sur la plateforme, propose plus de combats en permettant à Crash de donner des coups de poings et de pied à des ennemis, ... Le gameplay est aussi surtout axé sur le contrôle de Titans, des mutants géants beaucoup plus puissants que Crash lui-même. Ils peuvent être contrôlés par Crash en les chevauchant.
